Find your prescriber

You were entitled to a copy of your prescription at the end of the exam, free, without asking. If you never got one or lost it, the fix is a phone call: the office must provide a copy, and the script tool has the words to use. This page helps with the part before the call: finding the office. Search by the doctor's name or the practice name and you get the phone number and location on file in the federal provider registry.

Results come from NPPES, the National Plan and Provider Enumeration System, the federal registry that assigns every health care provider an NPI number. Providers keep their own entries current, so a phone number can lag behind a recent move. The registry confirms identity and contact information; it says nothing about license discipline, which lives with your state licensing board.
